Please fill out this request form and we will mail you a packet of 15 tickets or whatever number of tickets you indicate at the bottom of the form. 

Cost is still just $1 per Ticket. 

When you receive your tickets, just fill out the name and address section and return them to the retreat house. 

Once we receive your returned tickets, you will be entered into the Sweepstakes drawing with a chance to win:

2nd Prize:  $1,000.00 Cash
3rd Prize:   $500.00  Cash
